3. Fax the form and CERTIFICATE OF AUTHORITY TAX ID NUMBER to us at 1 (718) 874-0051. View the Sample Form Here
4. You will receive an email from us within 1 business days indicating that you are set-up as a WHOLESALE ACCOUNT.
5. You can then sign in and start using your WHOLESALE account site.

• Wholesale accounts are not eligible for any retail promotional deals, including, but not limited to, free shipping and item discounts.
• There are minimum purchase $1000 requirements.
